Hospice Care Certified Nursing Assistant CNA – Chester County (RF: 3107)
We are seeking compassionate and caring Certified Nursing Assistants CNAs for full-time and part-time positions providing hospice care throughout the Chester County, PA area. The CNAs will be providing personal care and light housekeeping services to terminally ill patients in home and facility-based settings. Candidates must be a PA Certified Nursing Assistant (CNA) and have at least one year of direct patient care experience in an institutional or home care setting. Candidates with community health/hospice experience are preferred.
Qualifications:
- Must be a PA Certified Nursing Assistant (CNA)
- Must have at least one (1) year of direct patient care experience in an institutional or home care setting
- Must have a HS Diploma or GED
- Must have a valid driver’s license and reliable transportation that is insured
- Community health/hospice experience is preferred
- Must have an understanding of Hospice philosophy and needs of terminally ill
Responsibilities:
- Provide personal care and light housekeeping services to terminally ill patients in the home, nursing facility, inpatient units or other environments where the Hospice patient resides in accordance with an established plan of care and under the supervision of a Registered Nurse (RN).
- Actively participate as a member of the Hospice Interdisciplinary Team (IDT).
- Provide care and comfort to patients in accordance with assigned tasks and established interdisciplinary and Hospice Aide plan of care.
- Observe the patient, report and document observations and care performed.
- Provide timely documentation of services based on the patient’s care plan
- Maintain a professional attitude and demeanor and is be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others.
- Demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and employees.
- Interact with all staff in a positive and motivational fashion supporting the mission and vision of the organization.
