Responsibilities:
- Establish and/or maintain a census report with the client expiration dates and staff lists
addendums, and memos necessary to ensure time restraints for authorization. - Coordinate with treatment team and staff as required to ensure continuity of care.
- Coordinate information to all team members regarding time frame for completion
of team meetings and submission of required documents to ensure timely submittal. - Ensure census report is updated with evaluation appointment and meetings.
- Timely notification to all team members of change in authoriation status and/or
reauthorization, with verification of the time frames for authorization. - Notification to parents and team members of eligibility issues, ensuring follow-up and
cease in service provision until problem resolved. - Update authorizations and client coordinate with staffing agencies information in
electronic health records programs. - Serve as a liaison between families and team members; provide support for questions,
concerns related to clients’ services.
